Treatments

The treatment of piles varies depending on severity of symptoms and grades of piles. The toilet advice and dietary changes are very helpful. The other treatments depends on the severity of the piles.

Injection Sclerotherapy

Injection sclerotherapy of haemorrhoids is a commonly done clinic treatment of haemorrhoids. 5% oily Phenol is injected into the base of up to 3 haemorrhoids. This stops the blood supply to the haemorrhoid and causes the haemorrhoid to shrink away over 3-4 days. It takes about 5 minutes and is usually well tolerated and effective in most patients. The procedure does not require anaesthesia and patient walks home immediately after the procedure in the clinic.

Banding

Banding of haemorrhoids is a simple procedure that takes less than 10 minutes. It can be done as an office procedure in the consultation room. It is indicated for haemorrhoids that are not fully prolapsed. Special equipment is used for this procedure in the clinic where by rubber bands are applied to the fleshy piles, inside the anal canal. Up to 3 rubber bands can be applied at one treatment setting. The procedure is usually painless and patients sent home immediatly. The strangulated piles will then slough off, with the rubber band, into the toilet in about 3 days, which the patients do not usually recognise and the haemorrhoidal symptoms settle down.

THD (Transanal Haemorrhoidal artery dearterialization)

THD (Transanal Haemorrhoidal artery dearterialization) is a minimally invasive treatment for haemorrhoids. It is performed in operation theatre under anaesthesia. It does not involve cutting of tissue and hence is expected to be less invasive with early recovery and getting back to work earlier.
The aim of the procedure is to identify the main artery feeding each pile using a special doppler kit and stitch it. This is a well tolerated procedure and is effective for piles where bleeding is the most bothersome symptom. For patients with prolapsing piles, we use of additional stitches to prevent them from prolapsing out of the anal canal. This is a day case procedure and patients are usually sent home on the same day.

Rafaelo Procedure

The Rafaelo procedure is a newer minimally invasive technique in which the surgeon will use the Rafaelo probe to deliver the radiofrequency energy to treat the haemorrhoid. The procedure takes about 20 minutes. It is expected to be less invasive and less painful with early return to work. These procedure are done as day case procedures with patient getting back home on same day most of the times.

Open haemorrhoidectomy

This is the traditional method of treatment which involves open excision of the haemorrhoids which is done for certain grade 3 and 4 haemorrhoids under anaesthesia. It is expected to take longer to recover from this procedure needing rest and pain control for few days at home before return to work. It is reserved for more severe and certain type of haemorrhoids.

 

Aftercare

Haemorrhoid treatment is usually carried out in clinic and sometimes in the operation theatre as a day case procedure, hence, you would not usually need to stay in hospital overnight. 

Clinic based treatments are usually quick, painfree and patients can go back to work afterwards on the same day.

You will be sent home with special advice on the preventative measures to minimise recurrence of haemorrhoids.
